      AND
      CPT codes for patient visit: 99201-99205, 99212-99215, 99241-99245, 99354-99355, 99385-99387, 99395-99397, 99401-99404
      AND
      Documentation in the medical record of a FEV1 40 % of predicted value
      AND
      Patient’s age is = 18 years of age
