Exclusions
A patient is not eligible or can be considered a denominator exception and excluded from the measure if one or more of the following reason(s) is documented at the time of the encounter:
Patient refuses to participate
Patient unable to complete questionnaire
Patient is in an urgent or emergent medical situation where time is of the essence and to delay treatment would jeopardize the patient’s health status
