Computed Tomography (CT) or Magnetic Resonance Imaging (MRI) Reports

  • Percentage of final reports for CT or MRI studies of the brain performed within 24 hours of arrival to the hospital for patients aged 18 years and older with the diagnosis of ischemic stroke or TIA or intracranial hemorrhage that includes documentation of the presence or absence of each of the following: hemorrhage and mass lesion and acute infarction.

Concurrent Use of Opioids and Benzodiazepines (COB)

  • The percentage of individuals >=18 years of age with concurrent use of prescription opioids and benzodiazepines during the measurement year.

    A lower rate indicates better performance.

Confirmation of Endotracheal Tube Placement

  • Any time an endotracheal tube is placed into a patients airway in the Emergency Department (ED)or a patient arrives to the ED with an endotracheal tube already in place ( via EMS or hospital transfer) there should be appropriate confirmation of ETT placement and documentation of its performance in the medical record.

Congestive Heart Failure Rate (PQI 08)

  • Admissions with a principal diagnosis of heart failure per 100,000 population, ages 18 years and older. Excludes cardiac procedure admissions, obstetric admissions, and transfers from other institutions.

    [NOTE: The software provides the rate per population. However, common practice reports the measure as per 100,000 population. The user must multiply the rate obtained from the software by 100,000 to report admissions per 100,000 population.]
