The percentage of patients 18 years of age and older who were discharged alive for acute myocardial infarction (AMI), coronary artery bypass graft (CABG) or percutaneous coronary interventions (PCI) during the 12 months prior to the measurement year, or who had a diagnosis of ischemic vascular disease (IVD) during the measurement year and the year prior to measurement year, who had each of the following during the measurement year.

1.14 Numerator
Patients who had a complete lipid profile performed during the measurement year and whose most recent LDL-C test during the measurement year had a result of <100 mg/dL.

1.15 Denominator
Patients 18 years of age or older by the end of the measurement year who were discharged alive for AMI, CABG or PCI during the 12 months prior to the measurement year or who had a diagnosis of IVD during both the measurement year and the year prior to the measurement year.
