30-Day Post-Operative Colon Surgery (COLO) and Abdominal Hysterectomy (HYST) Surgical Site Infection (SSI) Standardized Infection Ratio (SIR)

Annual risk-adjusted standardized infection ratio (SIR) of observed over predicted deep incisional primary and organ/space surgical site infections (SSIs), over a 30-day post-operative surveillance period, among hospitalized adults who are >=18 year of age with a date of admission and date of discharge that are different calendar days, and the patient underwent a colon surgery (COLO) or abdominal hysterectomy (HYST) at an acute care hospital or oncology hospital.  The 30-day postoperative surveillance period includes SSIs detected upon admission to the facility or a readmission to t

ACEI or ARB for left ventricular systolic dysfunction - Heart Failure (HF) Patients

Percentage of heart failure (HF) patients with left ventricular systolic dysfunction (LVSD) who are prescribed an ACEI or ARB at hospital discharge. For purposes of this measure, LVSD is defined as chart documentation of a left ventricular ejection fraction (LVEF) less than 40% or a narrative description of left ventricular systolic (LVS) function consistent with moderate or severe systolic dysfunction.

Adult CG-CAHPS Survey - Getting Timely Appointments, Care, and Information (Access)

The Consumer Assessment of Healthcare Providers and Systems (CAHPS) Clinician & Group Survey 3.1 (CG-CAHPS) is a standardized survey instrument that asks patients to report on their experiences with primary or specialty care received from providers and their staff in ambulatory care settings over the preceding 6 months. CG-CAHPS Survey Version 1.0 was endorsed by NQF in July 2007 (NQF #0005) and version 2.0 received maintenance endorsement in early 2015. Version 3.0 was released in July 2015 and was last endorsed in 2019.
