1. Infants outside the gestational age range of 22 to 29 weeks 2. Outborn infants admitted to the reporting hospital more than 28 days after birth 3. Outborn infants who have been home prior to admission 4. Infants who die in the delivery room or initial resuscitation area prior to admission to the neonatal intensive care unit 5. Infants not in the reporting hospital at the postnatal age recommended for ROP screening by the AAP
