Patients from the denominator receiving the following immunizations according to current AAP guidelines:
• DPT (DTP, DPT, DtaP, DTw-P-HbOC, DTwP-HIB, Acel-Imune, Tripedia, Infanrix, Tetramune [DTPH], Tripedia/ActHIB, TriHIBit, Certiva, Immunol)
• HepB (Comvax, Recombivax HB, Engerix-B)
• Polio (IPOL, IPV, OPV, Orimune, Poliovax)
• Hib (PedvaxHIB, HibTITER, ProHIBit [PRP-D], Tetramune [DTPH], Tripedia/ActHIB, TriHIBit, ActHIB, OmniHIB [PRP-T], Comvax)
• PCV (Prevnar, Pneumovax, Pnu-Imune).

Exclusions
Documented parent refusal and mortalities. The developer recommends that the measure be suspended when there are vaccine shortages rather than including vaccine unavailability as an exclusion.
