Greetings PQM Acute and Chronic Events Committee

I noticed that patient/carepartner behavioral voice is possibly missing within the roster. It seems important to have this perspective on the committee as the lack of access to care is so prominent in many communities today, patients and families and carers suffer. Issues around PHBH complex discharge and effective transitions are many where SdoH factors often add to the complexity. Developing an effective care plan where the BH patient co-creates the plan so that it is both realistic and attainable is important.
