Prenatal & Postpartum Care (PPC)

  • The percentage of deliveries of live births between November 6 of the year prior to the measurement year and November 5 of the measurement year. For these women, the measure assesses the following facets of prenatal and postpartum care:
    Rate 1: Timeliness of Prenatal Care. The percentage of deliveries that received a prenatal care visit as a member of the organization in the first trimester or within 42 days of enrollment in the organization.
    Rate 2: Postpartum Care. The percentage of deliveries that had a postpartum visit on or between 21 and 56 days after delivery.

Healthy Physical Development by 18 Years of Age

  • The percentage of children 18 years of age who had healthy physical development services. The measure has four rates: BMI Assessment, Physical Activity Counseling, Nutrition Counseling and Screen Time Counseling

Risky Behavior Assessment or Counseling by Age 18 Years

  • The percentage of adolescents with documentation of assessment or counseling for risky behavior by the age of 18 years. Four rates are reported: Risk Assessment or Counseling for Alcohol Use, Risk Assessment or Counseling for Tobacco Use, Risk Assessment or Counseling for Other Substance Use, and Risk Assessment or Counseling for Sexual Activity.

  • The percentage of adolescents who turned 18 years during the measurement year who had proper immunizations by the time they turn 18 years of age.

Risk-Adjusted Operative Mortality for Mitral Valve (MV) Repair + CABG Surgery

  • Percent of patients aged 18 years and older undergoing combined MV Repair and CABG who die, including both 1) all deaths occurring during the hospitalization in which the procedure was performed, even if after 30 days, and 2) those deaths occurring after discharge from the hospital, but within 30 days of the procedure
