The measure estimates a hospital-level risk-standardized readmission rate (RSRR) following elective primary THA and/or TKA in Medicare Fee-For-Service (FFS) beneficiaries who are 65 years and older. The outcome (readmission) is defined as unplanned readmission for any cause within 30 days of the discharge date for the index admission (the admission included in the measure cohort). A specified set of planned readmissions do not count in the readmission outcome.

Percentage of patients 18 years of age or older without carotid territory neurologic or retinal symptoms within 120 days immediately proceeding carotid angioplasty and stent (CAS) placement who experience stroke or death during their hospitalization for this procedure. This measure is proposed for both hospitals and individual interventionalists. This measure is currently reported by the Vascular Quality Initiative (VQI) Registry.

Percentage of patients age 18 or older without carotid territory neurologic or retinal symptoms within the one year immediately preceding carotid endarterectomy (CEA) who experience stroke or death following surgery while in the hospital. This measure is proposed for both hospitals and individual surgeons. This measure is presently reported by the Vascular Quality Initiative (VQI) registry.

Percentage of patients aged 18 years and older who had cataract surgery and had improvement in visual function achieved within 90 days following the cataract surgery

Percentage of patients undergoing elective endovascular repair of asymptomatic infrarenal abdominal aortic aneurysms (AAA) who die while in hospital. This measure is proposed for both hospitals and individual providers. The measure is currently reported in the Vascular Quality Initiative (VQI) Registry.

Proportion of ICD implant patients with a diagnosis of Left Ventricular Systolic Dysfunction who are prescribed beta-blocker therapy on discharge.

Proportion of ICD implant patients with a diagnosis of previous MI who are prescribed a Beta Blocker at discharge.

Percentage of patients aged 18 years and older with a diagnosis of nonvalvular atrial fibrillation (AF) or atrial flutter whose assessment of the specified thromboembolic risk factors indicate one or more high-risk factors or more than one moderate risk factor, as determined by CHADS2 risk stratification, who are prescribed warfarin OR another oral anticoagulant drug that is FDA approved for the prevention of thromboembolism

Percentage of patients aged 18 years and older with a diagnosis of nonvalvular atrial fibrillation (AF) or atrial flutter in whom assessment of all the specified thromboembolic risk factors using the CHADS2 risk criteria is documented

Percentage of asymptomatic patients undergoing open repair of abdominal aortic aneurysms (AAA)who are discharged alive. This measure is proposed for both hospitals and individual providers. At present, this measure is reported via the Vascular Quality Initiative (VQI) Registry.
