Percentage of patients 40 years and older with COPD exacerbations that are receiving a long acting bronchodilator

The CVD Risk Assessment Measure represents the proportion of pregnant and postpartum individuals assessed for CVD risk using a standardized tool at the clinic and facility levels of health care service delivery. The CVD risk assessment identifies pregnant and postpartum individuals without an existing CVD diagnosis who may have cardiovascular disease (CVD) or are at an increased risk of developing CVD, thus necessitating further evaluation. 

Medication Possession Ratio (MPR) for statin therapy in diabetic individuals over 18 years of age

Percentage of patient visits or patients with HF with assessment of clinical symptoms of volume overload (excess).

Percentage of patients who were provided with patient education on disease management and health behavior changes during one or more visit(s).

Percentage of patients with HF who also have paroxysmal or chronic atrial fibrillation who were prescribed warfarin therapy.

Percentage of patient visits for patients with HF with weight measurement recorded.
