CoreQ: Short Stay Discharge Measure

  • The measure calculates the percentage of individuals discharged in a six month time period from a SNF, within 100 days of admission, who are satisfied (see: S.5 for details of the time-frame). This patient reported outcome measure is based on the CoreQ: Short Stay Discharge questionnaire that utilizes four items.

Cultural Competency Implementation Measure

  • The Cultural Competence Implementation Measure is an organizational survey designed to assist healthcare organizations in identifying the degree to which they are providing culturally competent care and addressing the needs of diverse populations, as well as their adherence to 12 of the 45 NQF-endorsed® cultural competency practices prioritized for the survey. The target audience for this survey includes healthcare organizations across a range of health care settings, including hospitals, health plans, community clinics, and dialysis organizations.

Depression Response at Twelve Months- Progress Towards Remission

  • The percentage of adolescent patients (12 to 17 years of age) and adult patients (18 years of age or older) with major depression or dysthymia who are progressing towards remission by achieving a response (PHQ-9 or PHQ-9M score reduced by 50% or greater) twelve months (+/- 60 days) after an index visit.

Developmental screening using a parent completed screening tool (Parent report, Children 0-5)

  • The measure assesses whether the parent or caregiver completed a developmental screening tool meant to identify children at-risk for developmental, behavioral and social delays. Developmental screening is defined as a standardized tool that assesses the child’s risk for developmental, behavioral and social delays. The American Academy of Pediatrics recommends standardized screening using an approved screening tool as the best method of identifying children at risk for developmental, behavioral and/or social delays.

  • The ECHO is a survey that includes 5 multiple item measures and 12 single item measures:

    Multiple Item Measures:

    Getting treatment quickly
    -Get treatment as soon as wanted when it was needed right away

    How well clinicians communicate
    -Clinicians listen carefully

Fall Risk Management (FRM)

  • Assesses different facets of fall risk management:

    Discussing Fall Risk. The percentage of adults 75 years of age and older, or 65–74 years of age with balance or walking problems or a fall in the past 12 months, who were seen by a practitioner in the past 12 months and who discussed falls or problems with balance or walking with their current practitioner.
