DenominatorPatients with a diagnosis of atherosclerotic disease and whose most recent LDL-C is greater than 100 mg/dL.
ExclusionsGeneral exclusions:
1. Evidence of metastatic disease or active treatment of malignancy (chemotherapy or radiation therapy) in the last 6 months
2. Patients who have been in a skilled nursing facility in the last 3 months
3. Patients who are terminally ill or in Hospice
4. Patient or provider feedback indicating allergy, intolerance or other contraindication to lipid lowering therapy anytime in the past
