Advanced Illness and Post-Acute Care

With its focus on improving quality of life, advanced illness and/or post-acute care are distinct from care intended to cure an illness or condition, although it can be delivered concurrently with curative therapies. Advanced illness care focuses on medical care for people living with a serious illness. It aims to address the symptoms or stress related to the illness and improve the overall experience and/or quality of life for patients, caregivers, and family members. Advanced illness care may also include palliative and hospice care, which includes comprehensive care that addresses medical, emotional, spiritual, and social needs during the last stages of a person’s terminal illness.

 

Post-acute care includes medical or supportive care provided to individuals after leaving an acute care setting, such as a hospital, but they are not ready to return home. This includes rehabilitation and even palliative care services.

 

The Advanced Illness and Post-Acute Care committee considers measures that address post-acute care and advanced illness, including cancer and end-stage disease (e.g., end-stage renal disease). Subtopics may include advanced illness management, the assessment of physical, emotional, social, psychological, and spiritual aspects of care, access to and timeliness of care, patient and family experience with care, patient and family engagement, care planning, hospice care, palliative and end-of-life care, chemotherapy, rescue, and intensive care unit (ICU) care.

E&M Fall 2024 Advanced Illness and PAC Endorsement Meeting

The purpose of this meeting is for Recommendation Group members to discuss and share perspectives regarding the measures under endorsement review for their respective E&M committee. Endorsement voting occurs during these virtual meetings. Advisory Group members are invited to attend these Recommendation Group meetings to listen to the discussion. Measure developers/stewards are invited and encouraged to attend, as they will have the opportunity to respond to the Recommendation Group discussion during the meeting.

Home and Community-Based Services (HCBS) Consumer Assessment of Healthcare Providers and Systems (CAHPS®) Measures

The Home and Community-Based Services (HCBS) Consumer Assessment of Healthcare Providers and Systems (CAHPS) Survey consists of 19 measures that assess the experiences of Medicaid participants’ age 18 and older receiving long-term services and supports (LTSS). The measures report a case-mix adjusted top-box score. Measure scores are calculated based on participant responses to a cross-disability survey about their experiences with the LTSS they receive in the community, delivered through a Medicaid-funded HCBS program.

Home Health Care CAHPS Survey (HHCAHPS)

The updated Consumer Assessment of Healthcare Providers and Systems® (CAHPS) Home Health Care Survey, also referred as “HHCAHPS,” is a 25-item instrument. This is a standardized survey instrument and data collection methodology for measuring home health patients’ perspectives on their home health care in Medicare-certified home health care agencies. The survey is administered monthly to patients who recently received or are receiving home health care from Medicare-certified home health agencies. It is offered as mail-only, phone-only or mixed mode (mail with telephone follow-up).
