Proportion of Patients with Pneumonia that have a Potentially Avoidable Complication (during the episode time window)

Brief Description of Measure: Percent of adult population aged 18+ years with Community Acquired Pneumonia who are followed for one-month, and have one or more potentially avoidable complication (PAC) during the episode time window. Please reference the attached document labeled NQF_PNE_all_codes_risk_adjustment_12_14_15.xls, in the tab labeled PACS I-9 & I-10 for a list of code definitions of PACs relevant to pneumonia.

Rate of Timely Follow-up on Abnormal Screening Mammograms for Breast Cancer Detection

This electronic Clinical Quality Measure (eCQM) reports the percentage of female patients aged 40 to 75 years with at least one abnormal screening (BI-RADS 0) or screening-to-diagnostic (BI-RADS 4, 5) mammogram during the measurement period (i.e., calendar year) who received timely diagnostic resolution defined as either follow-up imaging with negative/benign/probably benign results or a breast biopsy within 60 days after their index (i.e., first) abnormal screening mammogram.

Rate of Timely Follow-up on Positive Stool-based Tests for Colorectal Cancer Detection

This electronic Clinical Quality Measure (eCQM) reports the percentage of patients aged 45 to 75 years with at least one positive stool-based colorectal cancer screening test (i.e., high-sensitivity guaiac fecal occult blood test, fecal immunochemical test, or Cologuard) during the measurement period (i.e., calendar year) who completed a colonoscopy within 180 days after their index (i.e., first) positive stool-based test result date.

The Risk Adjusted Post-Ambulance Provider Triage Emergency Department (ED) Visit Rate Measure (shorthand: Post-Triage ED Visit Rate Measure) assesses the quality of the triage and decision making by ambulance providers who transport low acuity patients to an alternative destination (non-ED location), or facilitate Treatment In Place (TIP), by identifying whether patients have a subsequent ED visit or death within three days.

RTCOM Composite Measure for Home- and Community-Based Services (HCBS) Recipients - Social Connectedness

This outcome measure assesses an individual's level of social inclusion and to what degree they are connected to others in their community. The target population for this measure is adults with disabilities who receive HCBS or HCBS-like services. This is a self-contained measure that can be administered independently of other RTCOM measures. 

The measure is administered through an in-person or video-conferencing interview where an interviewer guides an individual through a series of questions (i.e., items) on the measure. There are 14 items on this measure:

This instrument assesses an individual’s experiences in paid employment as well as their experiences searching for paid employment. The target population for this instrument is adults with disabilities who receive HCBS or HCBS-like services. This is a self-contained instrument that can be administered independently of other RTCOM instruments.
