This measure identifies moderate and severe complications among term infants who enter labor without preexisting fetal conditions (morbidity outcome measure). The scope of the measure includes both maternal and neonatal care and is appropriate for all levels of hospital care and is collected for a full year time period. The measure is designed to highlight processes of both obstetric and neonatal care that can be improved while recognizing that not all complications can be avoided. This is a clinical performance measure which assesses whether women age 65+ were provided appropriate treatment for urinary incontinence (UI). This measure has three rates:
(A) Assessment for UI: Percentage of female patients aged 65 years and older who were assessed for the presence or absence of urinary incontinence within 12 months.
(B) Characterization of UI: Percentage of female patients aged 65 years and older with a diagnosis of urinary incontinence whose urinary incontinence was characterized at least once within 12 months

Percentage of female patients aged 65 years and older with a diagnosis of urinary incontinence with a documented plan of care for urinary incontinence at least once within 12 months
Admissions with a principal diagnosis of urinary tract infection per 100,000 population, ages 18 years and older. Excludes kidney or urinary tract disorder admissions, other indications of immunocompromised state admissions, obstetric admissions, and transfers from other institutions.
[NOTE: The software provides the rate per population. However, common practice reports the measure as per 100,000 population. The user must multiply the rate obtained from the software by 100,000 to report admissions per 100,000 population.]
Assess the use of and the adherence of antipsychotics among members with schizophrenia during the measurement year
Proportion of inpatient hospitalizations for patients 65 years of age and older who receive an order for antipsychotic medication therapy.
The percentage of patients 5-64 years of age during the measurement year who were identified as having persistent asthma and who were appropriately prescribed medication during the measurement year.
Percent of patients undergoing isolated CABG who received an IMA graft.
