Percentage of eligible dialysis patients who complete a health-related quality of life assessment with or without assistance using the KDQOL-36 (36-question survey that assesses patients´ functioning and well-being) at least once during a calendar year.
Percentage of all adult (>=18 years old) hemodialysis or peritoneal dialysis patients prescribed an ESA at any time during the study period or who have a Hb <11.0 g/dL in at least one month of the study period for whom serum ferritin concentration AND either percent transferrin saturation or reticulocyte Hb content (CHr) are measured at least once in a three-month period for in-center hemodialysis patients, peritoneal dialysis patients, and home hemodialysis patients.
Percentage of patients aged 18 years and older with a diagnosis of community-acquired bacterial pneumonia with oxygen saturation documented and reviewed
Percentage of patients aged 5 years and older with a diagnosis of persistent asthma who were prescribed long-term control medication
Three rates are reported for this measure:
1. Patients prescribed inhaled corticosteroids (ICS) as their long term control medication
2. Patients prescribed other alternative long term control medications (non-ICS)
3. Total patients prescribed long-term control medication
Percentage of patients, age >=18 and <= 120 at diagnosis, who have their first diagnosis of cancer (epithelial malignancy), whose primary tumor is of the colon, and had at least 12 regional lymph nodes removed and pathologically examined for resected colon cancer

Percentage of patients aged 18 years and older with a diagnosis of nonvalvular atrial fibrillation (AF) or atrial flutter whose assessment of the specified thromboembolic risk factors indicate one or more high-risk factors or more than one moderate risk factor, as determined by CHADS2 risk stratification, who are prescribed warfarin OR another oral anticoagulant drug that is FDA approved for the prevention of thromboembolism
Percentage of patients aged 18 years and older with a diagnosis of nonvalvular atrial fibrillation (AF) or atrial flutter in whom assessment of all the specified thromboembolic risk factors using the CHADS2 risk criteria is documented
Percentage of adult in-center hemodialysis patients in the facility whose average ultrafiltration rate (UFR) is >=13 ml/kg/hour AND who receive an average of <240 minutes per treatment during the calculation period.
